Important notices for the eREB System:
1. Sub-forms may not automatically submit following last signature (September 10, 2025)
We are investigating reports that some sub-forms (e.g., renewals, amendments) are not automatically submitting after signature. In all reported cases, the research team has received an email that the signature has been applied. During this time, we recommend that research teams confirm that sub-forms have been submitted as expected.
We think that this might be related to a bug in the 'request signature' feature, so it is especially important to confirm submission if you are using this button. If you notice a failed submission, please contact Mirela at erebhelpdesk@hhsc.ca.
2. Some renewal reminders are not being sent as expected (September 10, 2025)
We are investigating reports that some renewal reminders are not being sent/received as expected. Research teams are reminded that continuing ethics approval must be in place for the duration of their study, regardless of whether the renewal reminders are received. Please ensure that renewals are submitted about 30 days prior to expiry to ensure no lapses in ethical oversight.
3. Parent form auto-submit is not working as expected (ongoing)
Normally, once the last signature is applied, the application will automatically submit. We have become aware that there are instances of applications (particularly GRA forms) not automatically submitting when the last signature is applied. We believe this might be related to the signature block in section 18.5 – if you are NOT obtaining a signature in this section, it may be more likely that your form will not auto-submit
